The staff at Madi’s Place are professional, compassionate and dedicated to the wellbeing of the youth they serve. The Child Care Supervisor at Madi's Place provides trauma-informed direct care services, ensures that day-to-day supervision of residents is consistent with agency standards and effectively works as a member of an interdisciplinary team.

The Child Care Staff serves as direct support for individuals with Developmental Disabilities within the Residential community-based setting. The primary responsibility is to create and maintain an environment where assigned individuals are safe and in compliance with Developmental Disabilities Administration guidelines and expectations. This is achieved through nurturing and caring for the individual with respect to their unique needs and desires. The staff should be in constant communication with their direct supervisor and agency Delegating Nurse (if applicable) to ensure all clients are receiving proper medical care to ensure the health of each client. The staff will assist individuals to participate in the community by accompanying individuals to medical appointments and activities.

The Delegating Nurse-RN coordinates care delivery, which will ensure residents’ needs are met in accordance with professional standards of practice through physician orders, community policies and procedures, and state and local guidelines. Provided care and manages clinical oversight of resident care within the Madi's Place community.

The Certified Medication Technician is designed for persons who will administer medication in the home or in assisted living facilities under the delegation of a RN.

A background check by Child Protective Services is required every 1 year for everyone who works with children.
Download and complete the Child Protective Services/Adam Walsh Background Clearance Packet.
Madi's Place's agency number for the form is NUMBER.
You will need to have your form notarized before submitting it to the State. (Learn more)

Get your completed paperwork notarized.
Then, submit your notarized CPS Background Check form online or at your local Department of Social Services (view full list of locations)
